What Is Surgical Penis Girth Enhancement?
Surgical penis girth enhancement is a surgical procedure for permanently increasing the circumference of the penis. Unlike penis lengthening – where existing internal length is relocated outward – this procedure changes the diameter of the penile shaft.
At UGRS Center Darmstadt, your surgeons use exclusively the patient's own fat tissue for this purpose. Foreign materials, implants, or synthetic fillers are not used. This decision is not only medically consistent – it also follows the current guideline recommendations of the European Association of Urology (EAU).

Why Autologous Fat – And No Foreign Material
The question of which material is used is decisive in penis girth enhancement. Various approaches exist on the market – from hyaluronic acid injections to autologous fat transplantation to silicone implants and other foreign products. Not all of them are equally safe. Some are not even approved for use in Europe (silicone sheets). The European Association of Urology warns against others in its current 2026 Reproductive and Sexual Health guidelines, because they are largely unresearched or even carry considerable risks. The EAU clearly advises against the use of grafts, silicone, and PRP plasma on the penis.
Names like Phallograft or the P-Shot often sound to patients like established or even innovative procedures, when in fact they are unresearched methods that the EAU clearly criticises in its 2026 guidelines.
We consider this kind of approach by physicians to be disreputable.
Alphafill, for example, is not a hyaluronic acid approved for use on the penis, even though its name may lead patients to assume otherwise.
The current EAU guidelines explicitly point out the risks of various foreign materials in penis-enlarging procedures. These include, among others, inflammatory reactions, rejection reactions, hardening, and, in the worst case, severe tissue damage requiring an elaborate corrective operation.
Why Autologous Fat Is the More Convincing Method
Avoiding foreign material is one of the most important advantages of autologous fat treatment: because exclusively the body's own tissue is used, the risk of rejection reactions is eliminated – in particular capsular fibrosis, which can occur with silicone or other implants. This capsular fibrosis often scars together with the penile nerve and is therefore risky.
With autologous fat, the result feels natural both to the touch and visually, and blends harmoniously into the body's own anatomy.
A significant portion of the transferred fat cells heal in permanently, so the result lasts over the long term. The partial breakdown of transplanted fat tissue during healing has been known for decades and is naturally taken into account when planning your surgery.
Not least, this is an established, scientifically grounded method. Prof. Alessandro Littara and Dr. med. univ. Roberto Melone, who both perform this procedure at UGRS Center to an equal standard, have made avoiding foreign materials a central part of their treatment concept since the very beginning of their careers.
Autologous Fat or Hyaluronic Acid – An Objective Comparison
Hyaluronic acid can be an option for men who initially want to try a non-permanent result or are looking for a minimally invasive introduction. UGRS does not perform this procedure – we focus on the permanent autologous fat method. An honest assessment of both approaches is, for us, part of every consultation.
No Glans Enlargement With Hyaluronic Acid
We have deliberately decided against offering this technique. We do not perform it. This method, which is often marketed to patients as harmless and safe, in fact carries considerable risks.
If the intended tissue expansion occurs outward rather than inward, an unsightly glans covered in spike-like nodules can result. These can practically no longer be removed, since each one would have to be cut away individually, leaving subsequent scarring.

How Penis Girth Enhancement Proceeds
Step 1: Fat Harvesting
The body's own fat tissue is harvested from a suitable site – usually the inner thigh or the lower abdomen. The choice of harvesting site is made individually and discussed with you in advance. The procedure at the harvesting site leaves only minimal traces.
Step 2: Processing the Fat
The harvested fat tissue is processed immediately after removal. In this step, damaged cells and other admixtures are removed to optimise the quality of the graft and maximise the rate of fat cells that permanently take hold.
Step 3: Introduction Beneath the Penile Skin
The processed fat is introduced evenly beneath the penile skin. The goal is a natural distribution that feels authentic both visually and to the touch. The entire procedure – in combination with penis lengthening – takes about 70 minutes.
Why More Is Introduced Than the Target
Part of the transferred fat tissue is broken down by the body in the first few weeks after the procedure – this is a well-known and factored-in biological process. For this reason, somewhat more fat is introduced from the outset than would be needed for the planned final result. The final, stable result becomes apparent after several months – not immediately after the operation.
Achievable Results
Based on our clinical experience, the following changes can be achieved:
- Girth increase: approx. 2 to 4 cm permanent
- Natural appearance – visually and to the touch
- Result applies to both the flaccid and erect state
As with this procedure in general, the figures given should be understood as guide values. How much the girth actually increases depends on several individual factors, which are discussed with you as part of the preliminary examination.
For the same reasons we do not perform glans enlargement with hyaluronic acid, we also do not perform it with autologous fat.
Shaft thickening that is not only functionally but also visually natural, with careful shaping of the transition to the glans, is one of the most demanding disciplines in penis enlargement surgery. A surgeon with many years of experience in this area can achieve impressive results.
Dents and asymmetries can occur with penis girth enhancement, but they are very rare with an experienced surgeon, as at UGRS.
The result is also determined by the rate of fat cells that permanently take hold, which can vary from patient to patient: since not all transferred cells heal in over the long term, this biological process has a direct effect on the final result.
There are, in principle, no guaranteed results in medicine, and we deliberately make no such claim. What we can guarantee you, however, is careful, individual planning, an experienced surgical team, and structured aftercare, all geared together toward the best possible result for your personal situation.
Autologous fat girth enhancement should not be performed with standard liposuction cannulas – their diameter is too large and could leave scars.
Who Is Penis Girth Enhancement Suitable For?
The decision to undergo penis girth enhancement should be well considered and based on realistic expectations. It is equally important that enough of the patient's own fat tissue is available for harvesting, since this forms the basis of the entire procedure.
Here too, the principle applies that expectations of the result must align with what is medically realistic if the procedure is to lead to genuine satisfaction. Not least, the patient's general health must allow for an outpatient procedure so that the operation can be carried out safely.
When We Advise Against Penis Girth Enhancement
We are equally open about when we advise against a procedure. A body fat percentage too low to provide enough harvestable material for a satisfactory result is one reason to advise against the operation. If a lack of fat tissue means no enhancement could be achieved at all, we categorically decline to perform the procedure.
The same applies when expectations of the result are unrealistic and cannot be reconciled with the anatomical possibilities. In such cases, it is our task to address this openly in conversation. Health-related contraindications can also rule out a procedure.

Combining With Penis Lengthening
Most patients opt for the combination of girth enhancement and lengthening. The reason lies in proportion: girth enhancement alone, without a corresponding adjustment in length, can make the overall appearance look unbalanced. Combining both allows for a coherent overall result.

Frequently Asked Questions About Penis Girth Enhancement
The portion of the autologous fat that permanently takes hold remains permanent. Since part of it is biologically broken down, somewhat more is introduced from the outset than would be necessary. The final, stable result is reached after several months – and is then permanent.
Yes. Because the body’s own fat tissue is used, the result matches natural tissue in consistency and feel. Many patients report that partners do not notice any difference from natural tissue.
Fat harvesting is carried out through very small incisions that are barely visible once healing proceeds normally. The exact harvesting site is discussed with you individually beforehand.
After five weeks, sport is possible again without restriction. In the first few weeks, we recommend taking it easy so as not to impair healing.
Since it is the body’s own fat tissue, the fat that has been introduced generally behaves like fat elsewhere in the body – meaning it can, in theory, react to significant weight changes. In practice, however, the effects are minor. Extreme weight loss should, where possible, not take place immediately before or after the procedure.

Surgical penis lengthening was long a subject hardly anyone spoke about publicly – neither patients nor doctors.
